What to Expect
To expedite your on-boarding process during your first visit to our clinic, you are welcome to download and complete the new patient forms to bring with you. Since a foundational component of integrative medicine is the partnership between provider and patient, our providers seek to gain a deep understanding of their patients’ symptoms and lifestyle or environmental factors that may have an impact on their wellbeing. To ensure this, our appointments typically take an hour.
